Dentre os biomateriais sint\u00e9ticos dispon\u00edveis no mercado, vale citar o \u03b2-tricalcio fosfato (\u03b2TCP) e a hidroxiapatita (HA), sendo considerados os melhores materiais cer\u00e2micos para a reconstru\u00e7\u00e3o \u00f3ssea.<\/p>\n<p style=\"font-weight: 400;\">O enxerto \u00f3sseo Plenum<sup>\u00ae<\/sup> Oss<sub>hp<\/sub> \u00e9 composto por uma biocer\u00e2mica bif\u00e1sica sint\u00e9tica contendo hidroxiapatita e \u03b2-tricalcio fosfato (HA\/70: \u03b2TCP\/30), potente em suas propriedades de osteocondu\u00e7\u00e3o e hidrofilia. A r\u00e1pida reabsor\u00e7\u00e3o do \u03b2TCP confere ao material um aumento da porosidade, prolifera\u00e7\u00e3o celular e angiog\u00eanese enquanto a lenta reabsor\u00e7\u00e3o da HA promove manuten\u00e7\u00e3o e estabilidade do volume \u00f3sseo por um maior per\u00edodo. Al\u00e9m disso, as caracter\u00edsticas estruturais do material possibilitam uma maior rugosidade e porosidade de sua superf\u00edcie que facilita a ades\u00e3o celular e garante ao Plenum<sup>\u00ae<\/sup> Oss<sub>hp <\/sub>alta atividade osteog\u00eanica promovendo a regenera\u00e7\u00e3o \u00f3ssea no defeito.<\/p>\n<p style=\"font-weight: 400;\">A Plenum<sup>\u00ae<\/sup> Guide \u00e9 uma membrana sint\u00e9tica \u00e0 base de polidioxano (PDO) com caracter\u00edsticas morfol\u00f3gicas e topogr\u00e1ficas similares a matriz extracelular sendo completamente reabsorvida e metabolizada pelo organismo. O uso de membranas reabsorv\u00edveis pode ser associado ao Plenum<sup>\u00ae<\/sup> Oss<sub>hp<\/sub> para regenera\u00e7\u00e3o \u00f3ssea guiada (ROG) promovendo uma barreira mec\u00e2nica no leito cir\u00fargico, permitindo a migra\u00e7\u00e3o de c\u00e9lulas do tecido \u00f3sseo circundante, a forma\u00e7\u00e3o \u00f3ssea, bem como a redu\u00e7\u00e3o do risco de complica\u00e7\u00f5es p\u00f3s-operat\u00f3rias a longo prazo.<\/p>\n<p style=\"font-weight: 400;\">Este estudo teve como objetivo avaliar a bioatividade do Plenum<sup>\u00ae<\/sup> Oss<sub>hp <\/sub>em associa\u00e7\u00e3o a Plenum<sup>\u00ae<\/sup> Guide na regenera\u00e7\u00e3o \u00f3ssea guiada em defeitos cr\u00edticos de calv\u00e1rias.<\/p>\n[\/vc_column_text][vc_column_text]\n<p style=\"font-weight: 400;\"><strong><em>Procedimento cir\u00fargico \u2013 Defeito Cr\u00edtico de Calv\u00e1ria<\/em><\/strong><\/p>\n<p style=\"font-weight: 400;\">Este estudo foi aprovado pelo Comit\u00ea de \u00c9tica em Uso de animais (CEUA 0128-2021), onde foram utilizados 10 ratos machos (<em>Rattus norvegicus albinus, Wistar<\/em>) com 3 meses de idade, pesando 300 g. Os animais foram sedados, e na regi\u00e3o parietal ao lado direito foi criado um defeito \u00f3sseo utilizando uma trefina com de 5 mm de di\u00e2metro interno sob irriga\u00e7\u00e3o com solu\u00e7\u00e3o isot\u00f4nica 0,9% de solu\u00e7\u00e3o de cloreto de s\u00f3dio. O osso parietal com 5 mm de di\u00e2metro foi retirado do interior do defeito, mantendo a integridade da dura-m\u00e1ter e o defeito foi preenchido com Plenum<sup>\u00ae<\/sup> Oss<sub>hp<\/sub> (250 \u2013 500 \u00b5m \/ 0,5g). Ap\u00f3s a aplica\u00e7\u00e3o do biomaterial, o Plenum<sup>\u00ae<\/sup>Guide (0,5 \u00d7 30 \u00d7 40 mm) foi personalizado e reduzido em segmentos de 10 \u00d7 10 mm para ser colocado sobre o defeito preenchido com a biocer\u00e2mica (Figura 1).<\/p>\n<p style=\"font-weight: 400;\">Os animais foram eutanasiados aos 7 e 30 dias ap\u00f3s o procedimento cir\u00fargico para a realiza\u00e7\u00e3o das an\u00e1lises histol\u00f3gica (colora\u00e7\u00e3o de HE), e imunoistoqu\u00edmica (anticorpos contra Col\u00e1geno1, Runx2, Osteopontina, Osteocalcina e TRAP) e aos 60 dias para an\u00e1lise de micro-CT.<\/p>\n<p style=\"font-weight: 400;\"><em>An\u00e1lise Histol\u00f3gica \u2013 Colora\u00e7\u00e3o de HE<\/em><\/p>\n<p style=\"font-weight: 400;\">Aos 7 dias ap\u00f3s a cria\u00e7\u00e3o do defeito \u00e9 poss\u00edvel observar a presen\u00e7a dos gr\u00e2nulos de Plenum<sup>\u00ae<\/sup> Oss<sub>hp<\/sub> envoltos por discreto infiltrado inflamat\u00f3rio, presen\u00e7a de vasos sangu\u00edneos e um tecido conjuntivo em organiza\u00e7\u00e3o. Em \u00edntimo contato com a biocer\u00e2mica, \u00e9 poss\u00edvel observar a presen\u00e7a de fibroblastos e c\u00e9lulas da linhagem osteobl\u00e1stica, ainda sem a presen\u00e7a de matriz \u00f3ssea. Aumento de 4x, 10x, 20x e 40x).<\/p>\n[\/vc_column_text][vc_column_text]\n<p style=\"font-weight: 400;\">Aos 30 dias observou-se moderada marca\u00e7\u00e3o de Runx2, indicando um aumento na presen\u00e7a de osteoblastos jovens neste per\u00edodo, especialmente pr\u00f3ximos ao biomaterial em degrada\u00e7\u00e3o. A marca\u00e7\u00e3o de OPN apresenta-se intensa na matriz extracelular aos 30 dias, especialmente na regi\u00e3o de \u00edntimo contato com o Plenum<sup>\u00ae<\/sup> Oss<sub>hp<\/sub>, indicando que o processo de mineraliza\u00e7\u00e3o est\u00e1 se desenvolvendo das bordas da biocer\u00e2mica em dire\u00e7\u00e3o ao centro. Da mesma forma, a marca\u00e7\u00e3o de OCN \u00e9 intensa aos 30 dias, indicando maturidade no processo de mineraliza\u00e7\u00e3o e grande quantidade de osteoblastos sintetizando a matriz mineral nas regi\u00f5es circundantes aos gr\u00e2nulos de Plenum<sup>\u00ae<\/sup> Oss<sub>hp<\/sub>. A marca\u00e7\u00e3o de TRAP apresenta-se moderada, indicando a presen\u00e7a de c\u00e9lulas multinucleares\/osteoclastos em atividade, especialmente nas regi\u00f5es de proximidade aos gr\u00e2nulos da biocer\u00e2mica. Vale destacar que a presen\u00e7a dos osteoclastos em atividade (TRAP), junto \u00e0 atividade osteobl\u00e1stica (Runx2, OPN e OCN) evidenciam as unidades multicelulares b\u00e1sicas (BMUs) mostrando a din\u00e2mica do processo de reparo \u00f3sseo, muito provavelmente estimulado pela presen\u00e7a dos biomateriais preenchendo o defeito \u00f3sseo (Figura 4).<\/p>\n<p style=\"font-weight: 400;\"><em>Aspecto Microtomogr\u00e1fico \u2013 60 dias p\u00f3s-operat\u00f3rio<\/em><\/p>\n<p style=\"font-weight: 400;\">Estes resultados preliminares mostram que a associa\u00e7\u00e3o de Plenum\u00ae Oss<sub>hp<\/sub> e Plenum\u00ae Guide atuam positivamente sobre o processo de reparo \u00f3sseo sendo uma excelente op\u00e7\u00e3o para procedimentos de reconstru\u00e7\u00f5es \u00f3sseas (Figura 5). As respostas celulares mostram que esta associa\u00e7\u00e3o de materiais promove o est\u00edmulo de marcadores de forma\u00e7\u00e3o \u00f3ssea no per\u00edodo p\u00f3s-operat\u00f3rio precoce e se mant\u00e9m at\u00e9 per\u00edodos tardios.
